In practical use a static form such as this has limitations. Suppliers often feed inputs through several processes, and a process may use several suppliers. To prevent confusion, select one of the categories as a base, and list the other categories multiple times. For example, list all the process steps in sequence, and write in the same supplier multiple times if it has inputs in multiple processes.

The purpose of the SIPOC Analysis Sheet is to paint a clear picture of how the suppliers and processes interact, and how to define the inputs of a process so customers receive consistent outputs.

We offer a free PDF version of the SIPOC Analysis Sheet to our guests, and a free XLSX version of the SIPOC Analysis Sheet to our registered users. (View the terms of use.) Don’t forget to look at all the other Lean forms we offer on our site.
