The basic steps to using the FMEA Worksheet are:

  1. Identify all failure modes.
  2. Consider the root causes.
  3. Assess the risk of the failure (severity, occurrences, and detection).
  4. Create an RPN (risk priority number) and rank the failure modes.
  5. Eliminate the risks.

We have provided you with a few versions of this FMEA form for your use. The free FMEA Worksheet on PDF is available to guests. An editable FMEA Worksheet on Excel is also free to registered users. (View the terms of use.)
