Traditionally an engineering tool, the FMEA Worksheet is also useful when assessing the risk of a Lean project.

The basic steps to using the FMEA Worksheet are:

  1. Identify all failure modes.
  2. Consider the root causes.
  3. Assess the risk of the failure (severity, occurrences, and detection).
  4. Create an RPN (risk priority number) and rank the failure modes.
  5. Eliminate the risks.
